Dear Parent/Caregiver, 

Does your child have a dental home where they have seen the dentist in the past 6 months? 

If not, hygiene services from a Registered Dental Hygienist can be offered to your child at school. 

Lincoln County Dental- Community Health Outreach Mobile Program (CHOMP) is offering professional dental cleanings, sealants, individualized dental education, fluoride applications and other hygiene services if you would like your child to participate in this program. 

OSHA and CDC training and guidelines will be applied to ensure the safest possible experience for all involved. Within the dental setting, full personal protective equipment, frequent hand washing, aerosol reduction techniques and all infection control disinfection procedures will be utilized. 

A basic visual screening will be provided as x-rays will not be taken; this is not an alternative to regular check-up or a full exam with a dentist. A dental report card will be returned home after care, informing you of the services your child received and should any concerns have been noted. After the procedure, an arrangement for a follow–up dental exam with a dentist should be arranged. If assistance in locating a dental home is needed, we can help.
